Supportive Care Needs Survey - Short Form 34
SCNS-SF34 is a 34-item survey of unmet supportive-care needs across physical, psychological, sexual, care, and information domains. This page is catalogue information. It is not a diagnosis.
Overview
What is the SCNS-SF34?
The Supportive Care Needs Survey Short Form 34 asks how much help a person needed in the last month. Each domain is scored from 0 to 100. The published form has no domain cut-off and no total. Responses labelled 4 or 5 are moderate or high unmet need for clinician review. Domain scores are not diagnoses and not a care plan.
Development and clinical use
Boyes, Girgis and Lecathelinais published the 34-item short form in 2009. Each domain score rescales that domain's item sum to 0-100. The paper publishes no domain cut-off.

Questions
What clinics ask before they evaluate the SCNS-SF34
These answers stay inside educational catalogue scope. They do not replace official manuals or clinician judgement.
Does a high need score diagnose a condition?
No. Moderate or high need on an item is a signal for clinician review of that domain.
Can I complete SCNS-SF34 here?
No. This page does not collect answers or return a score.
Must every patient receive all 34 items?
No. Use it when the team needs a structured view of unmet need. The clinician chooses the pack.
Does Lirena assign the next service from a domain total?
No. Returned forms stay with the responsible clinician. Lirena does not route care.
