Clinical Outcomes in Routine Evaluation - Outcome Measure
CORE-OM is the 34-item Clinical Outcomes in Routine Evaluation measure. This page is for clinics. It is not a public test and it does not diagnose.
Overview
What is the CORE-OM?
CORE-OM covers well-being (4 items), problems (12), functioning (12), and risk (6). It is longer than CORE-10 and is often used at the start and end of an episode.
Development and clinical use
CORE-OM was developed for UK psychological therapy services as a practice-based outcome measure. CORE System Trust publishes the form, the scoring information, and the system user manual.

Administration
A focused client experience
Clients complete all 34 statements about the last week. Eight positively worded items print the scores in reverse order under the same anchors. A complete form is required. The published pro-rating rule for up to 10% missing answers is not applied.
- One question at a time with visible progress
- Large keyboard-operable response choices
- Previous, next, and answer-review steps
- Clear confirmation that results go to the clinic
The illustration does not reproduce CORE-OM items.
Report showcase
Designed to make evidence easy to review
The downloadable synthetic example illustrates report layout using invented response patterns, without patient data or instrument questions.
- Named instrument, version, and provenance
- Item-level response pattern without source wording
- Methodology, references, and clinical boundary
- Responsible clinician review rather than automated conclusions
Clinician review required
Methodology
Understand the measure and its scoring conventions
The original score is the mean of the item scores, from 0.00 to 4.00. The clinical score is that mean times 10, from 0.00 to 40.00. The same mean is reported for well-being, problems, functioning, risk, and the 28 non-risk items. Table 5 of the CORE system user manual gives clinical versus non-clinical cut-offs on the mean scale. For men they are 1.37, 1.44, 1.29, 0.43, 1.36, and 1.19. For women they are 1.77, 1.62, 1.30, 0.31, 1.50, and 1.29. Gender is recorded so those cut-offs can apply. It is an office-use field on the form, not a numbered client item. Risk items are clinical flags for review when the printed score is 1 or higher. These cut-offs are not severity bands and not a diagnosis.

Questions
What clinics ask before they evaluate the CORE-OM
These answers stay inside educational catalogue scope. They do not replace official manuals or clinician judgement.
How is CORE-OM scored?
The mean of the 34 item scores runs from 0.00 to 4.00. The clinical score is that mean times 10. Table 5 cut-offs differ for men and women and apply once gender is recorded.
Can we use CORE-10 instead?
They serve different operational jobs. Do not mix scoring keys.
Can I complete CORE-OM here?
No. This landing does not collect answers.
What do the risk items mean?
Each risk item is a clinical flag for review when the printed score is 1 or higher. A flag is not a diagnosis.
